Ø       Full legal name & address of your company, including phone, fax and email address.

Ø       Language

Ø       The number of years your publishing house been in business

Ø       The number of titles you currently have in print

Ø       The number of titles translated from other languages

Ø       The number of books published each year

Ø       Sales/distribution territory

Ø       Channels of distribution (bookstores, direct mail, book clubs, etc.)

Ø       Names of other U.S. publishers with whom you have licensed

Publishing Plans

Ø       Title and ISBN

Ø       Number of copies you intend to print

Ø       Number of copies you anticipate selling in the first year
